Med., 1995, 175 (2), 77-90-We report three cases of extrahepatic portal-systemic encephalopathy (EHPSE), two of which did not show portal hypertension (PH) and in the other of which precirrhotic state with chronic active hepatitis was detected at liver biopsy. Our review of the literature regarding EHPSE without PH leads us to conclude that a part of the shunts is congenitally or spontaneously formed, that PH is not always essential to shunt formation. Since the clinical symptoms most often do not manifest until middle age, cases with only mild disturbance of consciousness are not easily distinguished from dementia. Furthermore, all reported patients underwent shunt closure followed by marked improvement of clinical symptoms and uneventful postoperative course. EHPSE without PH which can be corrected by shunt closure should always be differentiated from EHPSE with PH.

Med., 1995, 175 (2), 91-99-Fifteen patients underwent carinal resection for lung cancer during the years of 1957 through 1993. All were males with ages ranging from 40 to 81 (mean 60.1) years old. Cell Types of the cancer were squamous cell carcinoma in 12 cases, adenocarcinoma in 2 and small cell carcinoma in one. In 14 of 15 cases there was cancerous invasion into the tracheal carina (T4) and one had no involvement of the carina (T3). Four patients had no lymph node metastasis (N0), 2 had hilar node metastasis (N1), 8 had ipsilateral mediastinal node metastasis (N2) and one had contralateral mediastinal node metastasis (N3). Operative procedures for carino-plasty were sleeve pneumonectomy in 3 cases, pneumonectomy with wedge resection of the carina in 3, sleeve resection of the carina followed by reconstruction of the carina (Montage type) in 8, and carinal resection with reconstruction of the one-stoma type in one. Two patients died within one month due to pneumonia or renal failure (13.3%). Postoperative complications occurred in 14 cases and bronchorrhea (n=8), anastomotic stenosis (n=5) and pneumonia (n=4) were frequent. Excluding 2 operative deaths, 8 cases died from 2 to 37 months after operation (mean 13.1 months) and 5 are alive from 10 to 24 months without recurrences. Patients with squamous cell carcinoma with N0 or N1 had better prognosis.

    ZAVOS, P.M, CORREA, J.R. and ZARMAKOUPIS, P.N. Epididymal Spermatozoa: Recovery and Subsequent Improvements of Mouse Epididymal Spermatozoa via the SpermPrepTM Filtration Method. Tohoku J. Exp. Med., 1995, 175 (2), 101-109 -This study was designed to determine the effects of Sephadex filtration (SpermPrepTM method) on the separation of viable motile, morphologically normal mouse epididymal spermatozoa and to study the viability of the recovered spermatozoa over a 3 hr incubation period. Spermatozoa were harvested from the caudae epididymides (5 animals per run or replication; n=10) following bilateral testicular excision and incubated in 2-ml of Test-Yolk buffer (TYB) at 37°C for 15 min. The specimen was then split into 2 aliquots, with Alquot 1 as the control and Aliquot 2 used for filtration. SpermPrepTMI column was employed according to the manufacturer's specifications (ZBL, Inc., Lexington, KY, USA) using TYB. During filtration (10min), different fractions were obtained: first 5min (Sample 1) and second 5min (Sample 2). The filtered fractions were evaluated and incubated at 37°C and assessed for percentage and grade of motility (0-4) every 30min for 3 hr. Filtration resulted in a significant improvement in percentage and grade of motility (91.5% and 3.0 vs. 76.5% and 2.5, respectively). The results point out very clearly that the filtration via the SpermPrepTM method improved the percentage and grade of motility (p<0.05) but not the percentage normal morphology of the spermatozoa. Also the SpermPrepTMI enabled the recovery of 45% (8.3×106 spermatozoa) of the total spermatozoa processed in the control aliquot (18.4×106 spermatozoa) which is consistent with previous observations. Most importantly, filtered spermatozoa incubated for 3hr showed greater percentage and grade of motility than the control spermatozoa (63% and 1.66 vs. 39% and 0.82, respectively). This is of significant importance since the filtration via the SpermPrepTMI has selected not only greater quality spermatozoa but also spermatozoa with significantly long-term motility (longevity) during in vitro incubation.

    KATOU, F. and MOTEGI, K. Fixation of Mandibular Body Fracture with Combination of One-Piece Cast Splint and Miniplate Tohoku J. Exp. Med., 1995, 175 (2), 111-121-A method for fixation of mandibular body fracture within the dental arch using a combination of a rigid one-piece cast splint and a pliable miniplate screwed at the inferior border of the mandible was developed with the highest priority placed on restoration of occlusion. This paper presents the results obtained in 36 patients treated by this method. The postoperative occlusion was analyzed using the T-scan system in 20 fully dentate patients without major caries. The data obtained in the patients were well comparable to those in normal subjects.

A Clinical Trial of Therapeutic Electrical Stimulation for Amyotrophic Lateral Sclerosis. Tohoku J. Exp. Med., 1995, 175 (2), 123-134-This paper describes the effects of therapeutic electrical stimulation (TES) on the wasting muscles in a patient with amyotrophic lateral sclerosis. The patient is a 47-year-old male, and he has a history of progressive muscle weakness and atrophy, affected more in the right side. Percutaneously indwelling intramuscular electrodes were implanted to the affected muscles in the right upper and lower extremities but no electrode in the corresponding left region. Within a month of TES therapy, a rapid improvement of extremity motion appeared in the TES treated side. Long-term application of TES more than 3 months increased the strength of the muscle which had been evidently weaker than the non-treated side. CT findings of both the upper and lower extremities with TES therapy showed an increase in the density and a reduction in the moth-eaten image. An increase in the thickness of the muscles was also observed in the TES treated side while deterioration was observed in the muscles on the non-treated side.

Tohoku J. Exp. Med., 1995, 175 (2), 135-137-The relationship between maternal smoking and the head circumference of newborns were analyzed using a matched-pair longitudinal method. For the study, data was collected from Japanese primiparas in Aichi, Japan. Forty-seven smoking and nonsmoking mothers were matched using multivariate caliper matching for birth weight and gender for their newborns, and gestational period and age for the gravidas. Significantly smaller head circumferences of the newborns from the smoking mothers were observed.

Identification of Mycobacterium avium Complex Isolated in Eastern and Central Japan by Using DNA Probes. Tohoku J. Exp. Med., 1995, 175 (2), 139-142-An attempt was made to identify Mycobacterium avium and Mycobacterium intracellulare in the M. avium intracellulare complex (MAC) isolatad in the Tohoku (38 strains) and Tokai (30 strains) districts of Japan by using DNA probes which are specific for M. avium, M. intracellulare and M. tuberculosis complex, respectively. The incidence of M. avium infection (82%) by far exceeded that of M. intracellulare infection in the Tohoku district of eastern Japan. In the Tokai district of central Japan, the incidence of M. avium infection (57%) were slightly larger than that of M. intracellulare infection. Five of 68 strains showed a positive reaction with two different DNA probes. Thus a possibility of mixed infection could not be ruled out, because reference strains showed a positive reaction with only one species-specific DNA probe.
