Tohoku J. exp. Med., 1987, 152 (4), 319-324 - Isolated growth hormone (GH) deficiency (IGHD) is detected in 1/10 of pituitary dwarfism, but there are only a few reports on IGHD as an autosomal-dominant trait. We found one family with autosomal-dominantly inherited IGHD and examined their pituitary functions and GH genomes. Brothers (9.5 year and 11 year) and their mother (37 year) were diagnosed as having IGHD and their grandmother and uncle also seemed to have IGHD. All of their heights were under “mean-4.0 S.D.”. Cerebral tomograpy of brothers and their mother all showed “empty sella”, and GH-releasing hormone (GRH) tests showed no responses of GH not only to bolus intravenous injections but also after repeated intramuscular injections of GRH (100μg/day) for 7 days. Although genetic analysis (Southern blotting method) could not detect any mutations in their GH genomes, the IGHD lesion of them seemed to be pituitary in origin.

    SATO, S., TAKADA, K., NAKAMURA, H., YASUI, S. and TAKAHASHI, K. Elastic Recoil Pressure Arising from Surface Tension in Hamster Lungs Treated with Intratracheal Bleomycin. Tohoku J. exp. Med., 1987, 152 (4), 325-331 - Lung elastic recoil pressure arising from surface tension (Ps) was evaluated in normal and bleomycin-treated lungs. Twenty two male golden hamsters were separated into a control group (group, A, n=10) and a bleomycin group (group B, n=12). Group B was given a single intratracheal instillation of bleomycin and group A was given normal saline as a control. Thirty days after instillation, three cycles of static air -and saline-filled pressure-volume curves (P-V curve) were measured. Ps was graphically derived from air- and saline-filled P-V curves. The Ps of groups A and B were compared at the same lung volumes. Mean body weight and nose-to-tail length of the two groups did not differ significantly. Total lung capacity, defined as lung air volume at a transpulmonary pressure of 25cmH2O, was significantly smaller in group B (mean±S.E. was 4.08±0.20ml) than in group A (mean±S.E. was 5.23±0.12ml) (p<0.01). The Ps of group B was significantly larger than that of group A at all lung volumes studied (p<0.01-0.05). These results indicate that the lung elatic recoil pressure component due to the surface tension was increased by intratracheal instillation of bleomycin in hamsters.

Med., 1987, 152 (4), 333-338 - 4-Methylumbelliferyl-N-acetylglucosamine- 6-sulfate (4MUGLc6S) which is known to be a specific substrate for human hexosaminidase A was used to determine enzymatic features of canine GM2-gangliosidosis. The enzyme activity using 4MUGlc6S in affected dog brain and liver was less than 20 to 30% of control tissues, whereas total 4-methylumbelliferyl beta-glucosaminidase activity in canine GM2-gangliosidosis was normal or elevated. However, when beta-hexosaminidase was fractionated by DEAE-Sepharose column chromatography, beta-hexosaminidase A like fraction in affected dog tissues was reduced to 20 to 30% of control. These data suggest that canine GM2-gangliosidosis is analogous to human juvenile

A Simple Method for Quantification of Biotinidase Activity in Dried Blood Spot and Its Application to Screening of Biotinidase Deficiency. Tohoku J. exp. Med., 1987, 152 (4), 339-346 - A simple and reliable method for quantification of biotinidase (EC.3.5.1.12) activity in dried blood spot was devised by a modification of the colorimetric screening test developed by Heard et al. (1984). The enzyme reaction and hemoglobin denaturation were carried out in a U-bottomed microplate. An aliquot of the reaction solution was transferred to a flat-bottomed microplate. After the coupling reaction was started, the adsorbance was measured in situ by a microplate-reader. Both intra-and inter-assay coefficient of variation (CV) values were less than 10%. Biotinidase activity in dried blood spot showed a good correlation to that in serum (r=0.912, n=8). This method was applied in a pilot screening of 18, 945 newborns in Sapporo City. No positive results have been obtained as yet.

    KANOH, T. IgD (Kappa) Multiple Myeloma with Unusual Manifestations: A Unique Association. Tohoku J. exp. Med., 1987, 152 (4), 347-350 - The diagnosis of IgD (kappa) multiple myeloma was made in a 59-year-old woman. The disease was characterized by paraproteinemia of IgD (kappa) type, the occurrence of plasma cell leukemia, a polymorphous neoplastic proliferation with lymphoplasmacytoid cells, no Bence Jones proteinuria and a hemorrhagic pleural effusion. Judging from the related literatures, it seems likely that the following findings have a great tendency to occur in various combinations: IgD (kappa) paraproteinemia, no evidence of Bence Jones proteinuria, polymorphous features of myeloma cells and extramedullary spread.

Med., 1987, 152 (4), 351-362 - To assess the long-term effects of mineralocorticoids on the regulation of the synthesis or release of kallikrein and prostaglandin E2 in the renal kallikrein-kinin-prostaglandin E system, we studied the effects of chronic infusion of aldosterone (50μg/kg/day) on urinary excretion of total and active kallikrein, and prostaglandin E2 for 10 days in conscious rats on regular intakes of sodium and on sodium loading with 1% NaCl as a drinking water. Chronic infusion of aldosterone induced a prompt and transient decrease in the ratio of sodium to potassium and a sustained increase in urinary prostaglandin E2 excretion in rats on regular diets, whereas urinary total and active kallikrein excretion did not increase significantly until the 4th day of aldosterone infusion. In rats loaded with sodium, aldosterone did not induce any changes in urinary total and active kallikrein excretion, whereas it induced similar changes in the ratio of sodium to potassium and urinary prostaglandin E2excretion to those in rats on regular diets. Thus, the present results suggest that aldosterone might stimulate the synthesis or release of renal prostaglandin E2 independent of sodium balance. Furthermore, it is also suggested that aldosterone might stimulate the synthesis or release of kallikrein, at least partly, via the same pathway as sodium loading does.

Med., 1987, 152 (4), 363-374 - MC-838, calcium (-)-N-[(S)-3-(N-cyclohexylcarbonyl-D-alanyl) thio-2-methylpropionyl]-L-prolinate, is a new orally active angiotensin converting enzyme (ACE) inhibitor in which the mercapto-group is taken up in a stable thiolester linkage. The linkage was relatively resistant against enzymatic hydrolysis by rat liver homogenates. The ACE prepared from rabbit lung was inhibited by MC-838 in a concentration-dependent manner. In isolated rat aortic ring and guinea-pig ileum preparations, MC-838 was highly specific in suppressing the contractile response to angiotensin-I (A-I) and in augmenting the contractile one to bradykinin. However, the ACE inhibitory activity of MC-838 was 30-100 times less potent than that of captopril. In conscious two-kidney (2 KG), renal hypertensive rats and spontaneously hypertensive rats (SHRs), MC-838 (3 and 10 mg/kg) given orally caused a long-lasting hypotensive effect with a slow onset. When compared on a weight basis (3mg/kg), the antihypertensive effect of MC-838 was comparable to that of captopril in magnitude, but the duration of action of MC-838 was approximately 2 times longer than that of captopril.

Coagulability and Fibrinolytic Changes during Veno-Venous Bypass in Liver Transplantation. Tohoku J. exp. Med., 1987, 152 (4), 375-380 - In clinical liver transplantation, a venous bypass that rechannels the blood flow from the inferior vena cava and portal vein to the superior vena cava has been used to maintain the venous return. However, the usage of mechanical shunt has given rise to derangements of blood coagulability and fibrinolysis. Therefore, changes of coagulability and fibrinolysis during the venous bypass were examined in dogs using a centrifugal pump (Bio-Pump®), and the effect of gabexate mesilate for coagulation were studied. Venous bypass from the inferior vena cava and the portal vein to the external jugular vein was performed in mongrel dogs (group A: bypass without pump, group B: bypass with the Bio-Pump, group C: bypass with the Bio-Pump and the addition of gabexate mesilate). In group A, blood pressure gradually decreased, but in groups B and C it maintained the pre-bypass level. All results of coagulation parameters and fibrinolytic measurements of group B were not significantly different from those of group A; only partial thromboplastin time was prolonged in group B. However, this prolongation was effectively prevented by the addition of gabexate mesilate in group C.

    MITSUHASHI, T. Mechanical Production of Pleural Cysts in Isolated Rat Lungs: An Experimental Model for Pleural Blebs. Tohoku J. exp. Med., 1987, 152 (4), 381-389 - In an effort to establish the pathogenesis and developmental mechanisms of pleural blebs, which are not well understood, I attempted to produce bleb-like lesions experimentally in isolated rat lungs, and observed them histologically and ultrastructurally. One to 2ml of the Dulbecco's phosphate-buffered saline or a mixture of sodium citrate, citric acid and dextrose was injected in usual speed into the subpleural lung parenchyma while the hilum of the lung was lightly pinched with forceps. This treatment induced one of two kinds of pleural cysts near the site of injection; one, in which a space was produced between the pleural connective tissues and lung parenchyma, and the other, in which it was between mesothelial cell layer and the pleural connective tissues. These observations suggested that some of the blebs in humans may arise on mechanical stress, and that there may be two different types of blebs in humans, too.

Med., 1987, 152 (4), 391-395 -Isolation of the human immunodeficiency virus (HIV) was performed in 5 seronegative patients with primary immunodeficiency diseases (IDDs) who had no or little ability to produce antibodies and received a large amount of intravenous gammaglobulin (IVG) for a long period. In this study cultured cells were considered to be infected if the following 2 criteria were met: (1) They produced revers etranscriptase (RT) activity in the supernatant fluid and (2) expressed viral antigens. Under experimental conditions, virus was demonstrated by both RT activity and viral antigen expression detectable by immunofluorescence technique in lymphocyte cultures from 2 seropositive patients with acquired immunodeficiency syndrome-related complex who were examined as controls. There was no evidence of HIV infection in cultures of lymphocytes from all of the 5 patients with primary IDDs and from 2 healthy subjects. The above patients did not show the extremely low numbers of target T4+ cells, which make isolation of the virus in lymphocytes difficult. The present results suggest that current methods of IVG preparation either remove or inactivate HIV.

Med., 1987, 152 (4), 397-405 - The effects of somatostatin (SS-14) (0.1-10μg, i.c.v.) on brain histamine (HA) in male Wistar rats were investigated. HA was measured by HPLC with a cation exchanger and an automated fluorometric detection system. SS-14 induced time- and dose-dependent changes in the levels of HA in the anterior hypothalamus, posterior hypothalamus, median eminence, adenohypophysis, hippocampus, and frontal cortex. In the anterior hypothalamus, posterior hypothalamus and hippocampus, SS-14 (1μg, i.c.v.) significantly (p<0.01) decreased the levels of HA from 5 to 30min. This response was delayed by 5min in the frontal cortex and adenohypophysis. Only in the neurohypophysis, SS-14 increased the concentration of HA 10min after injection, but this response was not dose-related. These results seem to indicate that SS-14 influences neuronal HA probably modifying HA release, it leading to a decrease of the levels of the biogenic amine in specific areas of the brain. Since this effect is opposite to that developed by growth hormone-releasing factor (GRF) in identical conditions, HA seems to fulfil the criteria by which a neuroendocrine modulator should currently respond in a specific time- and dose-dependent manner to those neuropeptides which modulate. Therefore, HA is likely to play a key role as a neuromodulator of the somatotropinergic system in the rat
