    MUKADA, T. and YAMADA, S. Dysplasia and Carcinoma in Situ of the Exocrine Pancreas. Tohoku J. exp. Med., 1982, 137 (2), 115-124-To clarify the morphological aspect of precancerous and related lesions of the exocrine pancreas, histological studies were carried out according to a systematized protocol. The pancreas, its head, body and tail including the papilla and adjacent duodenal mucosa and the distal common bile duct from 206 unselective autopsy cases, excluding those of pancreatic carcinoma, were systematically examined. Histological grading of dysplasia was performed based on structural (SAT) and cellular (CAT) atypia which were evaluated by coding 0, 1, 2, and 3. Dysplasia and related changes were encountered in 75 cases (36%) including 6 carcinoma in situ, 1 occult invasive carcinoma (3%), and 10 moderate to severe dysplasias (5%). Of these 75, 46 were associated with parenchymal fibrosis but 29 were not. Simple epithelial hyperplasia and squamous metaplasia were observed in 90 cases (44%), and 34 cases (17%), respectively. Among 90 hyperplasias, 61 were associated with fibrosis, but 29 were not. The incidence of these epithelial abnormalities was higher than that reported in the previous papers. Both dysplasia and hyperplasia showed characteristic age, sex and site preponderances. An intimate relationship between dysplasia and chronic pancreatitis, and possible transition from dysplasia into carcinoma in situ and then invasive cancer were emphasized.

Med., 1982, 137 (2), 125-136 -The mechanical behavior of bronchial volume with respect to parenchymal properties, and to both the intrabronchial and pleural pressure, was investigated utilizing a theory of finite elasticity. Treating the lung parenchyma as a compressible continuum, we derived a simple strain-energy-density function from pressure-volume curves of saline-filled lungs. On the basis of this function, large deformations of the fluid-filled excised dog lobe could be analyzed by numerical procedures. For the purpose of obtaining peribronchial stress, the lung was represented by a hollow thick-walled cylinder corresponding to an axial bronchus with surrounding parenchyma. In general, we found that the theoretical results corresponded well to previous experimental results, being able to predict quantitatively the stress and strain around the bronchus during collapse previously demonstrated by Nakamura et al. Peribronchial radial and circumferential stresses were shown to be concentrated at the bronchial wall, but dissipated rapidly within 1-2 bronchial radii away from the wall. We conclude that the magnitude of regional lung recoil around bronchi during collapse can be plausibly estimated by a theoretical analysis of total lung pressure-volume relationships.

    TAZAWA, Y. and KONNO, T. Familial Cholestasis with Gallstone, Ataxia and Visual Disturbance. Tohoku J. exp. Med., 1982, 137 (2), 137-144-Two siblings with progressive intrahepatic cholestasis were reported. The brother died at 4 years of age because of hepatic failure followed by persistent obstructive jaundice starting at 4 months of age. The sister had unique clinical features, including recurrent obstructive jaundice since early infancy, radiopaque gallstone and neurological abnormalities which were cerebellar ataxia, bilateral ptosis, hyporeflexia and visual disturbance involving retinal degeneration and optic atrophy. She had a coarse facial appearance, camptodactyly and sclerotic skin with many scratch marks. Persistent high levels of serum bile acids were found while the patient was icteric and even anicteric, though serum cholesterol levels were approximately within normal limits. The serum lipoprotein-X was negative whenever examined. Cholestyramine treatment gave incomplete relief from pruritis but resulted in no improvement in her clinical course.

    SASAKI, K., INOUE, S., YOSHIDA, A., HAYASHI, F., MASUDA, Y. and HONDA, Y. Ventilatory Response and Drive Due to Carbon Dioxide Stimulation in Idiopathic Scoliosis. Tohoku J. exp. Med., 1982, 137 (2), 145-151-Ten patients with idiopathic scoliosis and seven controls were studied for the purpose of investigating the ventilatory response and drive due to CO2 inhalation by the steady state CO2 response test. To evaluate the ventilatory drive for the respiratory output mouth occlusion pressure (P0.1) was used. The ventilatory response to CO2 of the patients was not significantly different from that of the controls. P0.1 for a given PCO2 stimulation or tidal volume in the patients was significantly higher than the control. Ventilatory output against P0.1, however, was not different in the two groups. The small tidal volume of the scoliotic patients seemed to be compensated by the high respiratory frequency with a large ventilatory drive in order to maintain adequate ventilation.

    HARA, M., YAMAMOTO, T. and KIHARA, I. Effects of Injecting Brush Border Antigen on Heymann Nephritis. Tohoku J. exp. Med., 1982, 137 (2), 153-161-Autologous immune complex nephritis (Heymann nephritis) is mediated by the autoimmune mechanism and can be induced in certain rat strains by immunization with tubular brush border antigen (Fx1A). The present study was undertaken as a tentative approach to prevent this nephritis, by daily intraperitoneal injections of the solubilized Fx1A for the purpose of removing free antibody from the circulation. Administration of solubilized Fx1A was started a week after the immunization for the induction of Heymann nephritis and continued until the animals were sacrificed at 14 weeks. A control group of immunized animals, also observed for 14 weeks, received saline. The injections of the solubilized Fx1A decreased the incidence of proteinuria, the amount of immunoglobulin and complement depositions in the glomeruli, and also the titer of antibody against the brush border. It is concluded that the administration of the solubilized antigen exerts a preventative effect on the course of Heymann nephritis, probably due to removing the antibody from the circulation.

Med., 1982, 137 (2), 163-177-Peripheral T cells from patients with chronic active hepatitis (CAH) showed a significantly decreased suppressor effect (or increased helper effect) on allogeneic B cell differentiation into Ig-producing cells (Ig-PC) (p<0.05). After irradiation of T cells to eliminate suppressor influences, mean spontaneous helper activity of CAH was not different from that of healthy subjects, indicating that spontaneous helper activity of CAH was normal. Concanavalin A (Con A)-induced suppressor cell activity was significantly decreased in CAH (p<0.01, 9 defective cases out of 18 patients). Minor defect of Con A-induced suppressor activity was also found in some patients with chronic persistent hepatitis (CPH) (2 defective cases out of 14 patients). Autologous mixed lymphocyte reaction (AMLR) was significantly decreased in patients with CAH (p<0.005). Spontaneous suppressor or Con A-induced suppressor activity was not different statistically between HBsAg-positive and HBsAg-negative cases. Finally, we demonstrated a presence of a serum factor(s) that can decrease Con A-induced suppressor cell function of healthy subjects in 7 of 21 patients with CAH and 2 of 14 CPH. Our results suggest that defective suppressor cell function likely attributable to serum factor(s) may reflect altered immune responses of CAH.

Med., 1982, 137 (2), 179-190-To obtain a clue for the metabolic disorder of glycosaminoglycans (GAG) in orthopedic diseases, a screening study on excretion pattern of urinary GAG from orthopedic patients was performed by the procedures of Nagatsuka et al. (1980). All the urines examined gave three regular GAG-bands. Besides the regular bands, some samples gave irregular bands. Of 123 cases examined in the present study, the numbers of cases with an abnormal excretion pattern of urinary GAG in the regular bands (A) and with the irregular band(s) (B) were as follows: 23(A) and 7(B) in 38 cases of rheumatoid arthritis (RA); 8(A) and 5(B) in 13 cases of malignant bone tumor; 11(A) and 3(B) in 12 cases of benign bone tumor; 16(A) and 3(B) in 25 cases of bone metabolic and connective tissue diseases; 6(A) and 4(B) in 9 cases of osteaorthritis; 6(A) and 3(B) in 8 cases of spine diseases; and 14(A) and 8(B) in 18 cases of other orthopedic diseases. The excretion patterns of urinary GAG suggested that the metabolic rates of chondroitin sulfates tended to elevate in most cases of (A). Also, certain metabolic disorder of dermatan sulfate and heparan sulfate was suggested in several cases of (A). In addition, the abnormal metabolism of acidic glycoprotein(s) was suggested in many cases of (B). Excretion of a large amount of dermatan sulfate was elucidated in one case (Case 20 of bone metabolic and connective tissue diseases) by digestion with chondroitinases after the screening study. Therefore, the metabolic disorder of dermatan sulfate was indicated in this patient. The low activity of α-L-iduronidase in her urine supported this indication. This patient was finally diagnosed as mucopolysaccharidosis I-S (Scheie syndrome).

Tohoku J. exp. Med., 1982, 137 (2), 191-198-It is well known that the administration of glucocorticoids inhibits the activities of hypothalamus-pituitary-adrenal axis. The present studies were undertaken to define the site of inhibition of both corticosterone and dexamethasone on this axis in rats. Infusion of 20 or 202μg corticosterone over 6hr did not change the ACTH content in the anterior pituitary and plasma ACTH levels even though the infusion markedly inhibited the hypothalamic CRF content in normal rats. The constant infusion of a relatively small dose of 20μg dexamethasone inhibited the plasma ACTH level, but not the ACTH content in the anterior pituitary and the hypothalamic CRF content. In contrast to the above results, the infusion of 202 and 504μg dexamethasone over 6hr increased the ACTH contents in the anterior pituitary, but significantly reduced the hypothalamic CRF content and plasma ACTH levels. These results suggest that the mechanism of dexamethasone inhibition in the hypothalamus-pituitary-adrenal axis is the reduction of ACTH release rather than that of ACTH production in the anterior pituitary.

    NEMOTO, R., KATO, T., MORI, H., IWATA, K. and HARADA, M. Hyperthermic Irrigation of Urinary Bladder Tumor in Rabbits. Tohoku J. exp. Med., 1982, 137 (2), 199-205-Hyperthermic treatment for bladder tumor in the rabbit was investigated by the irrigation with hot saline through cystostomy after the transplantation of V2 carcinoma cells. When the bladder was irrigated with 42-44°C saline, the inhibitory effect on transplanted tumor growth in the urinary bladder was significant in comparison with control groups. Antitumor effect on the survival and changes in body weight were also significant. Histological study showed that tumor cells were more susceptible to heat than normal mucosal layer. The results suggest that the hyperthermic irrigation of the urinary bladder was effective in preventing the tumor growth with minimum damage on the normal bladder mucosa. Moreover, this malignant bladder tumor of rabbit seems especially suitable for the purpose of studing a new method for transurethral hyperthermic treatment of the urinary bladder cancer.

Med., 1982, 137 (2), 207-212-Neutrophil function was studied in prematures, mature newborns and school children. In premature infants, chemotaxis and killing of neutrophils were decreased, although superoxide production of neutrophils was normal. In healthy full-term infants, only chemotaxis was abnormal. The ability of neutrophil phagocytosis was within normal limits both in premature and mature newborns.

Med., 1982, 137 (2), 213-220-A 48-year-old man, who was diagnosed as adult-type citrullinemia by quantitative estimation of urea cycle enzymes in the liver, showed a regular nocturnal rise of blood ammonia level. In order to elucidate the mechanism of diurnal fluctuation of blood ammonia level, the patient was put into fasting state for four days. The blood ammonia level rose at the night of the first fasting day even though no food was taken, then it decreased gradually and reached the lowest level in the morning of the third fasting day. Intravenous administration of amino acids mixture under the same starved condition gave rise to a significant elevation of blood ammonia level. Based on these results, it was concluded that hyperammonemia of adult-type citrullinemia could result from the accumulation of free ammonia which was produced from the catabolism of amino acids absorbed from the small intestine and surpassed the urea synthesis of defective urea cycle to flood into the blood. Furthermore, the rise of blood ammonia level at the night of the first fasting day suggested that the circadian rhythm of amino acid-carbohydrate metabolim might superimpose on the process mentioned above.

    TANNO, Y. and TAKISHIMA, T. Enhancement of Mitogen-Induced Human Lymphocyte Transformation by Diiron Enneacarbonyl in Serum-Free Medium. Tohoku J. exp. Med., 1982, 137(2), 221-227-Diiron enneacarbonyl, colloidal iron particles used for removing monocytes, enhanced the 3H-thymidine uptake into the mitogen-stimulated human lymphocytes in serum-free medium, whereas neither silica particles, latex particles, carbonyl iron powder nor iron pentacarbonyl did. Because monocytes had no influence on the enhancing effect or diiron enneacarbonyl did not bind to PHA, it was suggested that the enhancing effect was performed by diiron enneacarbonyl directly acting on the mitogen-activated lymphocytes in serum-free medium.

Tohoku J, exp. Med., 1982, 137 (2), 231-232-99mTc pertechnetate imaging of the stomach was carried out in a case of 63-year-old man 15min after amogastrin. A coldness on the scintigram led to an exploratory laparotomy, and a carcinoma in situ was found and operated.
