The Effect of Ursodeoxycholic Acid on Biliary Bile Acid Composition in Patients with Cholesterol Gallstone. Tohoku J. exp. Med., 1982, 136 (3), 235-249 - To elucidate the role of conjugated biliary bile acids in gallstone dissolution, the acids in bile were determined by high-performance liquid chromatography before and after the treatment with ursodeoxycholic acid for 3-26 months in patients with gallstone. The stone-dissolving effect of ursodeoxycholic acid was confirmed in 7 of 10 patients and the lithogenic index lowed significantly after the treatment. The compositions of cholate, chenodeoxycholate and ursodeoxycholate were about 33, 45 and 4%, respectively, in the control and pre-treatment groups. In the post-treatment group, a markedly low value was observed in primary bile acids both glycine-conjugates and taurine-conjugates, especially in cholate, with a significantly high value of ursodeoxycholate (p<0.001) of both conjugates. On the other hand, no difference was observed in the composition of deoxycholate with significantly low percentage of taurine-conjugates compared with that in the pre-treatment group. The ratio of glycine- to taurine-conjugated bile acids showed a significantly higher value in the post-treatment group than in the pretreatment group (p<0.001) and the control group (p<0.005). The bile specimens were measured concomitantly by gas-liquid chromatography and the results were compared with those of high-performance liquid chromatography. The mean value of total bile acids, the ratio of cholate to chenodeoxycholate and the ratio of glycine- to taurine-conjugated bile acids obtained by the former analysis procedure represented about 57, 80 and 115% of those of the latter. It is concluded that the high G/T value seems to have a role in the dissolution mechanism.

    HYODO, S., KATAHIRA, S. and SHIGETA, S. Experimental Infection of Immunocompromised Mice with Achromobacter xylosoxidans. Tohoku J. exp. Med., 1982, 136 (3), 251-261 - The natural resistance of mice to Achromobacter (A.) xylosoxidans was decreased to 1/80 by a single dose of 250 mg of cylophosphamide (CY) per kg intraperitoneally. When mice were infected with 1×109A. xylosoxidans, the numbers of organisms in the liver., spleen, kidneys, lungs and heart blood reached to more than 106 per g at 6hr after infection, and then decreased to less than 104 at 96hr after infection. However, in the CY-treated mice, the numbers of organisms in these organs began to increase rapidly at 6 hr after infection and exceeded 109 per g at 48hr when the mice died. Seventeen of 18 immunocompromised mice and 23 of 24 normal mice which were infected with 1 to 2×109 organisms of A. xylosoxidans, respectively, were still living until 4 weeks after infection when they were killed and examined. Thirteen of the 33 surviving mice which were examined for bacteriological assay were chronically infected with A. xylosoxidans, some in the spleen, others in the kidney, lung, brain and/or heart blood but none were infected in the liver. Most of the infected and surviving mice had produced an antibody against A. xylosoxidans and its titer was higher in the bacteriologically “cured” group than in the chronically infected one. All surviving mice were resistant to the 2nd challenge with a lethal dose of A. xylosoxidans even when they received an effective dose of CY. The surviving mice showed activated cellular immunity and rather depressed humoral immunity as compared with normal mice or the mice which were infected with killed A. xylosoxidans.

    SEKINE, T. and WOODWARD, E.R. Significance of Sustained Gastric Hypersecretion by the Emulsified Histamine Solution. Tohoku J. exp. Med., 1982, 136 (3), 263-266 - Experimental production of gastric and duodenal ulcers was investigated in dogs by administration of histamine emulsified in Freund's incomplete adjuvant. Acid secretory responses were also checked. The results are as follows: Intramuscular administration of 10mg/day of the emulsified histamine solution caused sustained gastric hypersecretion due to gradual liberation of histamine but produced no apparent side effects. However, neither gastric nor duodenal ulcer was observed within 2 months with the dosage of 10mg/day. These results suggest that intramuscular administration of the dosage of 10mg/day within 2 months would be a valuable means to elucidate the pathogenesis of ulcer under the sustained gastric hypersecretion.

    TAKAHASHI, H. and WAKASA, H. Angioimmunoblastic Lymphadenopathy: Light and Electron Microscopic Studies Tohoku J. exp. Med., 1982, 136 (3), 267-284 - The present study evaluates the significance of clinical and histologic features in 16 cases of angioimmunoblastic lymphadenopathy (AIL). The disease observed in the elderly is characterized by an acute onset of constitutional symptoms, generalized lymphadenopathy, hepatosplenomegaly, and immunologic abnormalities. The clinical course is rather short, and autopsied cases show severe infectious complications. In the present study, two histologic types were identified: 1) Angio-cellular (A-C) type; the lymph nodes contained lymphocytes, immunoblasts, histiocytes, eosinophils and/or plasma cells. 2) Immunoblastic and plasma cell (I & P) type or lymphocyte depleted (LD) type; the lymph node contained mainly immunoblasts and plasmocytoid cells. Serum protein was examined in 12 cases. Dysproteinemia was noted in 5 of 6 cases of A-C type and in 5 of 6 cases of I & P type. There is a definite relationship between histologic types and survival periods. The A-C type is an expression of slight depletion of lymphocytes and the I & P or LD type represents marked depletion of lymphocytes. AIL has malignant potentiality, though it resembles graft-vs-host reaction in histology, the wide-spread involvement of tissues and clinical features. An immunologic investigation on 3 cases suggests that the disease is based on B-cell disorder.

    GOTO, K., SASANO, N. and MATOBA, N. Follicular Reconstruction and Hormone Production by Human Adenomatous Gioter Cells in Culture. Tohoku J. exp. Med., 1982, 136 (3), 285-290 - Morphological and functional properties of dispersed cells of human adenomatous goiter and those of the thyroid tissue adjacent to and distant from the nodular lesion (normal control) were investigated. In the presence of TSH, reconstructive arrangement of adenomatous goiter cells into a three-dimensional follicular structure occurred in a similar manner to that of normal controls. An addition of thyrotropin (TSH) to culture resulted in the secretion of thyroid hormones in control cells, but adenomatous goiter cells showed no response to TSH. It was found that the ratio of rT3, to T3, the ratio of an inactive to an active form, was about three times higher in adenomatous goiter than in normal control. These findings suggested that conversion from T4 to rT3 is increased in adenomatous goiter.

Histologic Findings of Ductus Arteriosus and Pulmonary Vascular Disease in Interrupted Aortic Arch. Tohoku J. exp. Med., 1982, 136 (3), 291-298 - There are still a number of unsolved problems concerning the total correction of interruption of the aortic arch. Here we report a histopathological investigation of the patent ductus arteroous and pulmonary vascular disease using autopsy material of a patient with Celoria type B interruption of the aortic arch who died postoperatively. The so-called ductus arteriosus between the pulmonary arterial trunk and the descending aorta was lacking in elastic fibers and smooth muscle cells at the media showing discontinuous, transverse fibrous tissue. The vessel walls were weak and normal development was apparently impossible. Consequently, it was concluded that in the reconstruction of the aortic arch in such cases with PDDT, it is advisable not to use the patent ductus arteriosus. In this case, hypertrophy of the media of small pulmonary arteries was marked, but pulmonary vascular intimal lesions were mild. With regard to the pathogenesis, it is thought that there was sufficient thickening of the media of the pulmonary arterial system in response to the pulmonary hypertension present from birth, and it prevented the progression of the intimal lesions by accomodating to the intraarterial pressure.

    UCHIYAMA, Y. The Membrane Association among the Rough- and Smooth-Surfaced Endoplasmic Reticulum and Golgi Apparatus in Rat Hepatocytes at Certain Circadian Stages. Tohoku J. exp. Med., 1982, 136 (3), 299-302-Subcellular structures of the cytoplasmic components in perilobular hepatocytes were examined at 0500h and 1700h in 6 adult male Wistar rats per time period. At both time points, the endoplasmic reticulum (ER) having smooth and rough parts could clearly be observed. Corresponding to the decrease and increase of the smooth-surfaced endoplasmic reticulum (sER) during the 24-hr span, it is thought that the membrane transformation from the sER to rough-surfaced endoplasmic reticulum (rER) at 0500h and from the rER to sER at 1700h occurs in the flowing-off of membranes which is accompanied by attachment or detachment of ribosomes. Furthermore, at these time points, the direct continuity of membranes between the rER and Golgi apparatus is discerned. From these findings, it may be elicited that the membrane association among the cytoplasmic components takes an important role in the reorganization of the cytoplasmic components in hepatocytes during the 24-hr span.

Med., 1982, 136 (3), 303-312 - Rabbit antisera were raised for red cell stroma extracted with each of the following solutions; 5mM phosphate buffer pH 8.0 (A antigen), 5mM EDTA containing 5mM 2-mercaptoethanol (B), 1/15M phosphate buffered saline (C), 0.05% SDS (D), 50mM Tris-HCl buffer pH 8.0 containing 1% SDS, 1mM EDTA and 1% 2-mercaptoethanol (E) and 1% Triton X-100 (F). Crossed immunoelectrophoresis showed that E antigen produced relatively numerous precipitation lines, 7-9, against each of the antisera, suggesting that E solution was superior to the others for membrane solubilization. However, the Ouchterlony test demonstrated potent precipitin activity in anti-C, and the antiserum absorbed with monkey stroma which was extracted with F solution was proved to have strict human specificity. By adopting the antiserum to affinity chromatography, human specific antigens were isolated. The antigens gave four bands moving faster than Band 5 at SDS-polyacrylamide gel electrophoresis none of which was stained by Schiff's reagent. Using absorbed anti-C, human specificity of blood stains kept up to for two years could be identified.

    ISHIKAWA, K., KANAMASA, K., YAMAKADO, T. and KATORI, R. Reduction of Left Ventricular Epicardial Segment Length by 100% Oxygen Breathing in Open-Chest Dogs. Tohoku J. exp. Med., 1982, 136 (3), 313-318 - We conducted this study in order to learn whether or not oxygen inhalation reduces left ventricular size, one of the major determinants of myocardial oxygen demand. In 11 open-chest dogs, a Mercury-in-Silastic gauge was applied to measure left ventricular circumferential length while the dogs were being ventilated with either room air or 100% oxygen. Four characteristic notches were identified on the resulting length curve: L1, length at the beginning of ejection; L2, length at the end of ejection; L3, length in early diastole; and L4, length at end diastole, L1 was shortened from 24.9±10.5 to 24.4±9.9mm (a decrease of 1.4±2.1%) by oxygen breathing, L2 was also shortened from 26.8±11.5 to 26.2±10.7mm (a decrease of 1.5±2.9%), L3 from 17.5±4.4 to 17.4±4.3mm (a decrease of 0.7± 2.7%) and L4 from 17.7±4.8 to 17.5±4.7mm (a decrease of 1.3±2.4%). These changes all disappeared when the inspiratory gas was changed from oxygen back to air. Heart rate and left ventricular end-diastolic pressure showed no significant changes but were accompanied by a slight reduction in aortic pressure and myocardial contractile force. This study demonstrated a small but consistent reduction in left ventricular circumferential length by oxygen breathing. This reduction in left ventricular size will reduce left ventricular tension and thus result in reduction of left ventricular myocardial oxygen demand when oxygen inhalation is applied to patients with ischemic heart disease.

Med., 1982, 136 (3), 319-322 - A rare variant (-D-/-D-) in two generations due to consanguineous marriage was reported. The propositus (T.M., 27 years of age) delivered her second baby who suffered from severe jaundice. She had agglutinin of titer 1:64 in saline and 1:1, 024 in Coomb's test against red cells of her husband (CDe/cDE). Family study revealed that T.M's father and his two siblings were grouped as -D-/-D-. Each of the siblings (both females, 53 and 46 years of age) had three children who were delivered normally without hemolytic disease. No agglutinin concerning the Rh-Hr system was detected in the sera of the siblings.

    ABE, S. Observations with Endocardial Electrography on Ventricular Tachycardia by Inrracoronary Saline in Dogs. Tohoku J. exp. Med., 1982, 136 (3), 323-335 - Selective injection of 1.5ml of 10% saline into the left circumflex coronary artery (LCX) was carried out during sinus rhythm twice in each of 15 mongrel dogs anesthetized with sodium pentobarbital, and multiple endocardial recordings including His bundle electrogram (HBE) were made. Ventricular tachycardia (VT) was elicited in all of the 15 dogs both in the first and second injections. The cycle length of sinus rhythm before injection was 468±88msec (mean±S.D.). The coupling interval between the last depolarization of the left ventricle (LV) of sinus origin and the first premature LV depolarization of the VT was 395±57msec. Coupling interval between the first and the second LV depolarizations was 348±78msec. Significant difference was noted between any pair of these values (p<0.01). No bundle branch re-entry in the beginning of VT was observed. The site of origin of the VT was referred to the LV in 13 dogs. Rapid pacing during paroxysm of VT applied at the apex of the LV made no marked influence upon VT after pacing. The calcium channel blocker, diltiazem failed to block the production of VT. The mechanism for genesis of this VT may not be re-entry but rather enhanced automaticity independent of the calcium inward current.

Computed Tomography in the Diagnosis of Fatty Liver: Total Lipid Content and Computed Tomography Number. Tohoku J. exp. Med., 1982, 136 (3), 337-342 - Fifty-three histologically proved cases of various diffuse liver diseases were studied for their computed tomography numbers (CTN). The machine used was Ohio Nuclear's Delta Scanner 50FS type and CTN was expressed by the Hounsfield unit (H). Mean CTN in each group was as follows: 66.6±2.6H in normal control (N), 63.3±6.0H in chronic hepatitis (CH), 61.8±7.0H in liver cirrhosis (LC), and 44.4±10.6H in fatty infiltration (FI). There were no significant differences among them except FT group. As N group were all above 60H and CH and LC groups were all above 50H, CTN below 60H suggests chronic liver disease or fatty infiltration and CTN below 50H strongly suggests fatty infiltration. In eleven cases where total lipid content of the liver could be biochemically determined by the sulfo-phospho-vanillin reagent, a relation of total lipid content to CTN was studied. As a result, a significant correlation existed between them (r=-0.89; p<0.001). If the diagnostic criterion of fatty liver was set at total lipid content above 100mg/g wet liver, CT criterion was estimated at CTN below 48H from the regression formula. - fatty liver; computed tomography; computed tomography number; total lipid content

Tohoku J. exp. Med., 1982, 136 (3), 343-348 -The effect of two kinds of caloric intake, 30kcal and 60kcal per kg of ideal body weight with the same amount of protein, on glucose tolerance and the metabolism of thyrotoxicosis was studied. A diet of 30kcal per kg of ideal body weight was not sufficient for the thyrotoxicosis, and all the patients lost body weight. On a diet of 60kcal per kg of ideal body weight, weight reduction was minimal and the urinary nitrogen and blood lactate levels increased. Insulin secretion also increased on a 60kcal diet, but there was no deterioration of glucose tolerance even in cases of thyrotoxicosis showing mild glucose intolerance. These results suggest that sufficient calories for the thyrotoxicosis are at least 60kcal per kg of ideal body weight, irrespectively of mild glucose intolerance.

Med., 1982, 136 (3), 349-350 -A new synchronous video urodynamics to evaluate the function of the lower urinary tract is described. The urodynamic study together with the transrectal longitudinal ultrasonotomography of the bladder and urethra can be seen on the colour display monitor through the display controller. An image on the colour display monitor is also stored on the video cassette recorder. This new technique will be indispensable for evaluation of the lower urinary tract dysfunction.

Med., 1982, 136 (3), 351-352 The localization of postganglionic neurons to the penis within the major pelvic ganglion of the male rat was investigated by means of the retrograde HRP method. The results demonstrated that labeled neurons are localized in the caudal pole of the ganglion and also in the main branch to the penis and that some labeled fibers pass through the ganglion into the pelvic nerve.
